my new workout journal format

I've been thinking of the best way to log my workouts and bring my planned workouts to the gym. I do bring my phone to the gym to use for music, but I hate to keep looking at it so I've decided to use a good old fashioned faux moleskine as my log. 

It will go something/exactly like this:

I will put a smiley face if I got sore from any strength workouts and a sad face if I do not, so I know whether to amp up the reps or add/increase weights. 

Do you use your phone to workout at the gym? How about your iPad? These are things that I don't mind using at home but at the gym it feels funny. I'm really curious to know how you feel about it!
